Appointments

To book an appointment, all clients must provide a valid credit card number. All clients must complete the required forms online before their initial appointment with your clinician. All required documents must be completed 24 hours before the appointment.

Returning Clients: If you have not been seen in six months or more, you will be required to complete new Informed Consent and Credit Card Authorization forms.

Cancellation Policy: If you do not show for your scheduled appointment or cancel less than 48 hours (two business days) notice, you consent to be charged and are required to pay $130 for the first missed session/late cancellation. Afterward, you will have to pay $75 for all missed/late canceled appointments. Any testing appointments missed/late canceled are charged at $130. To cancel or reschedule an appointment, you must contact our office (210-384-1254. Email (info@monarchbehavioral.net) or your clinician directly. Insurance companies do not pay for late cancellations and no-shows. It is entirely the client’s responsibility.

*NOTE: We DO NOT provide custody evaluations. We DO NOT provide psychiatric services or medication management.

 

Insurance

Our office cannot guarantee insurance coverage for any service provided. It is important to contact your insurance provider. Check with them about your specific coverage since every plan is different. All copayments, coinsurance, and deductibles are due at the time of service.

If we do not accept your insurance plan, we will provide you with a SuperBill at your request. You can submit this to your insurance company for reimbursement consideration.

If your insurance does not pay or cover your services, you are responsible for the balance due. Please check your coverage by calling your insurance provider directly.

Our providers accept various insurance plans. These include but are not limited to:

  • Multiplan

  • Some Medicaid and Medicare plans

Please note that not ALL providers are considered in-network for ALL insurances. Please let us know if you are requesting a certain provider and we will let you know if they accept your insurance.

Therapy Fees

Doctorate Level Clinician Fees: $190

Licensed Master’s Level Clinician Fees: $130

Provisionally Licensed Associate: $90

Behavior Consulting Services: $120

Student Practicum Services: $40

EMDR Sessions: $180

These fees apply to both in-person and telehealth services. Session lengths vary. Your clinician will tell you the length of your sessions. A client who leaves a session early will be charged for the entire session.

Our clinicians have several spaces for lower-cost therapy rates via Open Path. Potential clients search for the Monarch clinician and submit the form through Open Path to obtain this rate.

*NOTE: These rates do not apply to practicum students, behavior consulting services, or evaluation services.

Evaluation Fees

**All evaluation fees include an intake session, a written report, and typically a feedback session. All evaluations are completed and/or reviewed by a licensed psychologist.

Comprehensive Evaluation $2,800

Commonly used to identify autism spectrum disorder, cognitive delays, neuropsychological disorders, and learning disabilities. Includes but is not limited to cognitive/intelligence, academic, memory, attention, language, and processing testing, and behavioral, emotional, adaptive, and social assessments as needed.

Developmental Evaluation $980 (under age 5) / $1600 (age 5 and up)

Commonly used to identify intellectual disabilities and other developmental delays. Includes but is not limited to cognitive/intelligence, academic or academic readiness, memory, attention, language, and processing testing, and behavioral, emotional, adaptive, and social assessments as needed.

ADHD Evaluation $1300

Includes but is not limited to cognitive/intelligence, academic or academic readiness, memory, attention, and processing testing, and behavioral, emotional, adaptive, and social assessments as needed.

TPAPN Evaluation: $1000

Law Enforcement / Pre-Employment Evaluation: $1000

School / Exam Accommodations Testing: $1750

An evaluation tailored to meet high school, college, entrance examinations, and other board exam requirements. (i.e., MCAT, SAT, LSAT…) *NOTE: We cannot guarantee any accommodations will be granted by the school or board / entity.

Intelligence and Academic Testing $200 per hour

Includes test administration, scoring, and a brief report with no diagnosis or recommendations.

School Entrance Testing: $175 - $500 (varies by school’s requirements)

Includes a brief report with no diagnosis or recommendations.

Consultation Fees: $150/hr

Fees

Notice to clients and prospective clients:

Under the law, Section 2799B-6 of the Public Health Service Act, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. 

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. 

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate. You can do so before you schedule a service, or at any time during treatment.  

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
